I'm currently wearing one of those and it does not stick out like that under the clothes unless they are very tight fitting like a t-shirt. The pads in the back are very soft. The side to the body is metal but the rest is pockets of smurf gel. It molds quite nicely to your back and is fitted inside a pouch.

She may have a "heavier" version but as far as I know there is only one.
