and to make a diagnosis as a medical professional, you need to examine the patient, not base your decision off of news clips. But you would know that, being the doctor you are.

That’s not strictly true if, by “examine” you mean sitting in the same room as the patient and having some kind of hands-on contact.

One very common example is the fact that when a patient gets a diagnosis for some kind of psychiatric condition and is referred to a specialist for some kind of treatment, the insurance company has their own MDs on staff who review the patient’s medical records and prescription and has to approve or disapprove insurance coverage of the treatment plan. Those MDs who review and approve the treatment plan never come near the patient and certainly don’t have any hands-on contact. They simply look at the notes and lab results, if any, on which the primary physician made their diagnosis and agree or disagree. When they do that they are making a diagnosis.

The second thing to consider is that Psychiatric or Psychological disorders are by definition behavioral disorders. The way they are diagnosed is by observing behavior of someone. That is distinct from internal medicine, where you can’t just look at someone and tell whether they do or do not have high blood pressure, for example. All the Psychiatric and Psychological professionals who are now publicly discussing Trump’s and Biden’s psychological conditions relative to each other are actually examining their publicly available words and actions.

As an example, you can look at video recordings of Trump and Biden from 10 or 15 years ago and compare them to recordings from last week. What you see is that Trump has a much smaller vocabulary now, very often repeats himself, forgets words, makes up nonsense words, speaks in incomplete sentences, and often will veer off onto a tangent in the middle of a sentence. Biden will often be recounting some memory and give the wrong name to an actor in his story.

Dr John Gartner is probably the most outspoken medical professional on this:

“This is becoming a weekly ritual: A round-up of the latest behaviors evidencing Trump’s apparent dementia. For the eighth time, Trump announced that he was running against Obama. No one believed it when he said he was joking the first seven times, and he keeps saying it, showing just how deeply disoriented he is and how advanced his apparent dementia has become.

Trump is continuing to show more of these phonemic aphasias: “Venezuero” instead of Venezuela. He is also demonstrating semantic aphasias: “steak mountain or steak hill,” instead of “Snake mountain.” Trump is continuing to slur words. What is even more troubling is how Trump sometimes can’t form words at all but just makes sounds. For example, “Saudi Arabia and Russia will…. bluh-ub-bll….”

Dr. Gartner’s observation is that Biden is exhibiting the behavior that goes along with an aging brain, Trump is exhibiting behavior of a dementing brain.

Many people who have never had any formal training in Psychology probably can’t tell the difference between the two conditions.

The US being the "richest country in the world" has nothing to do with individual wealth. The 1 percenters are worth more than most of the rest of the US population will ever even see combined.
