My take:
Like many other vain world leaders (that's you Slivio B, skulking at the back there !)
Putin has succombed to a host of plastic surgery interventions
Botox, rhinoplasty, browlift, blepharoplasty, facelift...

That would be why he doesn't go out in the sun, smile too much

Puffy cheeks have made his eyes appear "smaller".

It seems to be common in famous men as they age their faces swell unlike most mens faces which tend to thin out as they age. Look at Joe rogan, Dana white from ufc their heads have grown bigger & balder! maybe it is HGH or similar. Must be the latest thing for rich people to have big bald round heads 'the neanderthal look'?!

It would surprise me if a world leader/elite type person didn't have body doubles, clones, robotoids etc. Throughout all these countries having wars, corporate conflicts the most important thing is to keep the status quo so the people don't become troublesome or uncooperative which could upset the balance of power, carefully created by the hidden powerful elites.
On first watching 'WestWorld from the 1980's' i assumed it was pure make belief sci-fi, but on watching 'WestWorld from 2017' i can see how it could relate to our present understanding of reality! Hypothetically if true i wonder who & from where world events are being dictated/decided i assume for the United Nations to implement?
