the first thing that comes to my mind is suicide, he thought it would be quick and painless but after the first didnt kill him he was too scared itd happen again so he went to a place he knew hed get an xray

Here's a front-on X-ray. Seems it went through his cheek. Seriously, the nail must've killed something in his brain for him not to notice this.
