Is there true 'Heath Insurance' anymore?

I'm talking about the type where you pay for all normal health care yourself and the insurance is only for a major medical. (Get hit by a car, shot in crossfire, true accidents that are not common but do happen)
If anyone knows of such a system let me know. I am not talking about 500$/month if you are the 1/20,000 who need such care they might claim they will pay 70% after you pay the first $10,000. then deny the claim when it is time to write the check.
