That and rouge terrorist groups. There are so many nukes or even just materials for dirty bombs now that's it's only a matter of time until some psycho gets a hold of something bad and decides to make people pay attention. Full scale nuclear assault isn't likely because there isn't an outcome which is good for anyone. You'd have to be completely suicidal to start it because there is no winning it. Everyone loses. But a single terrorist with a nuke or dirty bomb, much more likely.
