LONDON — Britain's foreign secretary says an inquiry into the death of a British aid worker in Afghanistan confirms she was killed by a U.S. grenade.William Hague told lawmakers Thursday that U.S. special-forces soldiers have been disciplined after the joint U.S.-U.K. investigation into the incident found that a failure to immediately disclose that a grenade had been thrown breached military law.

Well it looks like it has been confirmed after all that time then. I know a few threads went around a while back about this but it was a lot of speculation.

What are the ramifications of this? Why were the US using grenades in such a close environment? Bad intelligence possibly telling them which room she was in and got the room wrong?
