Look in any war and chance always plays a massive part. Falklands war is a classic:
HMS Sheffield was "off-line" at the time of the attack as the Admiral was phoning home
Argies bombed too low -hence althouth they hit many targets, their explosives did not go off (as the detonator did not have time to count down)
Argies hit the most important container ship containing landrovers and helicopters (hit as the missile deflected from one of thw warships).
