Nevertheless, Gudea's plans for the temple of Ningirsu indicates that such a concept as an architectural plan did exist in the ancient world.

It was a ratio of building and room lengths,widths and heights that was the basic starting point to any project.

Recently watched video on pyramids summit blocks. The inner blocks arn't aligned with perimeter blocks suggesting a spiral ramp structure was indeed used. But the maker of the vid suggests that the pyramids were built in stages. Building on top of older structures.

The video suggests they built a core then built the rest around that core. That is the mainstream view.
That's not to say they built over older structures.

More like some did, the Mesoamericans were rather famous for that, the Egyptian did so also but not on a regular basis, Sumerians renewed the mudbrick but sure if that counts. You'd have to do a case by case study for that one.
