Had she not had schleroderma, she would not have gotten pneumonia.

That's true for most deaths outside of accidents. Most people who die have more than one issue. The doctor has the choice of which issue they want to list as being responsible for the deaths. List the blunt force trauma from a car wreck and maybe get a small percentage of the bill, or list the Chinese virus and get paid in full. Choices, choices...
