The only issues, as shown, were with two manufacturing facilities not compliant with FDA requirements and falsified records, missing records, etc. at those two facilities. See particularly the "Questions and Answers" document from FDA provided above.

and did the watered down drugs come from those facilities?

The answer to that would be yes they did and we know this because of the reporting on the medications having irregularities, leading up to the info I posted.

Further, as you can see from the info, they knew issues were present starting in 2002 and they did not stop using the company as a source until 2010.
