Some reports suggest the cleaning water mixed with chemical residue inside the exchanger, producing a hydrogen blast.

The plant manufactures polymers and uses a number of highly volatile chemicals including hydrogen and chlorine.

Japan: Fatal blast at Mitsubishi Materials factory

Is this a concern for the environment or just another indicator that Japan is dumb with this high tech stuff lol.

I mean they put a nuclear power plant near the sea when their country is famous for earthquakes.

Japan isn't the only country to have a nuclear power plant by the sea, and it's shocking how many of them are also on known fault lines.
Industrial accidents are also very common worldwide. I won't even ask why this one is so deserving of the media's attention, and not ignored like most.
Safety doesn't always seem to be a priority to big businesses.
