Will I be accused of a crime and arrested if I speak out about the government?

Looking at what the Obamacare law entails...yes, you will be fined if you do not have a GP. Digging in more, a lot of GPs are just going to retire because they don't want to deal with this Byzantine/Labrynthian type of situatiton. They will not get paid the same.

I think there are certain topics people are "allowed" to speak out about. Things like taxes, gun control, abortion, and gay marriage are pretty much safe topics to have an outspoken opinion about (whatever side you are on). This gives people the illusion they have freedom of speech. Veer off these selected topics and start to get an actual following and you might end up in trouble.

Also, if you have some personal info regarding people in power or start to do your own research which exposes government abuses, then yes, be very careful.
