The Indian military doesn't have the greatest record for safety. Their Air Force has lost a number of older aircraft in the last few years, as well as pilots.

The Sindhurakshak is a Kilo class submarine, built in Russia. India doesn't have the capabilities to overhaul them, so when they require major maintenance, or an overhaul, they have to send them back to Russia. INS Sindhurakshak underwent an overhaul beginning in 2010, and underwent sea trials in October of 2012. But even with Roboronservice Ltd setting up offices in India to give better support to the Indian Navy, the Indians have problems getting parts to maintain their submarines.
Spare parts have always plagued the Indian DOD. Conterfeit and/or inferior/used quality spare parts are a problem. Even the russians are known to send faulty/used spares at times despite collecting a hefty amount. Not to mention obscene levels of corruption within the political system and the money thats made by contractors and politicians alike in the name of spare parts and components for many of their weapons systems. However Safety Rules have always been a problem as well. Based on the color of the flames (blueish white and then yellow), more than likely 'hydrogen' or some sort of gas explosion followed by other fuel sources (explosives, fuel etc). Most of the time the rules are made only to be broken such is the mindset in general amongst many in that region IMO.

It sounds to me that after following the thread here? India needs to have a chat with Russia about a refund of services on the last refit and some compensation for lost men and damaged families from those who died aboard the boat.

Submarine service is among the most secret and dangerous in the world's militaries. That shouldn't be among the most dangerous while the boat is sitting at the DOCK though.

My condolences for the loss of your countrymen.

“If it was sabotage, only an insider could have done it”

While the jury is out on what really triggered the crippling blasts that wrecked the naval submarine INS Sindhurakshak in Mumbai on Tuesday night, chances of a sabotage causing it are giving the Navy the jitters. Given that the fully-armed submarine was at the high-security Naval Dockyard in preparation for a long patrol on Wednesday evening, only an insider could have done some foul play, if that was the case, indicate Navy sources. The severity of the twin explosions that gutted the forward area of the kilo-class submarine, which was handed over to India after a $ 150-million upgrade in January this year, points to torpedo burst, as the flame bore an unmistakable orange hue, sources say.
