Miitary Intelligence, Oxymoron?

OK, firstly, MODS if this is in the wrong section please move it for me.

Down to business.

A large number of us here on ATS are in or was in the Military. During my service I had to rely on intelligence of many forms, whether it be forward observers, RADAR, radio-chatter intercepts or AWACS.

And naturally things can and do happen that can be put down to horrible intelligence (un-telligence) from Intel.

What I'm getting to is this; What examples have happened to you personally or to someone you know (please no famous incidents that is already known - I'm interested in the ground level stuff) due to lapses in Intelligence of any form in the Military.

I'll start it off with one I got from a close mate in the Australian Army.

During his Initial Employment Training the platoon were to use Smoke Grenades to emulate 'Phos. Bombs.' Now thses guys being recruits wouldn't know the difference (let alone READING THE GRENADES PRIOR). The section commander actually gave out Phos. Bombs, and I guess you can imagine the many cases of burns in teh base hospital later that day.
