Yes it is in your head; chemical signals in the mammalian part of the brain that keep on going on and on. ER doctors are only to do he first handling of incoming patients. Ask any psychiatrist or neurologist about brain chemistry and they'll tell you.

There is a free database of medical articles on the net, forgot the name. There you can read up on stuff that only the specialists know.

Someone else said it already, but I agree with them totally. This sounds a lot like PTSD. My advice is to go talk to a psychiatrist and see if you can't work it out somehow. They will know the PROPER medications for whatever you have, a regular doctor really does not understand the brain very well.
As to what happens when you have a serious anxiety attack,
When I used to get them so bad that I couldn't stop shaking, I would find my favorite music, a stick and then flip the stick up and down in my hand to the beat of the music until the anxiety went away. It is all pure distraction until the symptoms go away.
