I would expect more attention to detail, and a greater command of the English language from a medical doctor; I would imagine patients would expect it as well.

Possibly the reason the bill was so high was because the person had no insurance. They may be expecting them to default and they can claim it as a tax loss or file a huge bill to the govt to be reimbursed.
