It is my understanding that it is not a common objective to 'wipe out' whole cities, killing innocent civillians, but to bomb only military and government installations and facilities...


Actually, you're talking about the concept of 'Total War,' in which a nation's populace is deliberately targeted. Also, Cold War strategy was calculated in megadeaths.
