You're making it more complicated than it really is:

If you live in Japan, it's a big concern, and the closer to the disaster you live, the bigger concern it is.

If you don't live in Japan, it's not a big concern. Without going through the detailed math, I'm estimating the chances of death in the US from Fukushima will be on par with the chances of death from a lightning strike. Some people will die from each cause, but it will be a relatively rare event.

The death rates in Japan will be much higher, especially for the workers at the nuclear plant.
