burn the tips of your finger tips so they no longer have prints. Go out of the country and get all your teeth replaced with caps (pay cash) and give them a fake name. Over the course of a couple months you need to take your own blood a little at a time till you have about 6-8 pints and make sure you are keeping all of it in the fridge. When it comes time to go all you have to do is park your car somewhere secluded throw your wallet in the passenger seat and pour all your blood into the car.
Congratulations, you will be "dead", although they did not find your body when they find 8 pints of your blood they will assume you are dead. but of course check your local laws, where i am it is legal to fake your death as long as it does not effect anyone financially good or bad.

There are no half-in/half-out solutions to going underground. Like we say down south... "either you is, or you ain't."
I have considered just such contingency planning and after careful consideration, I realized that the only real solution is to disappear... just drop out of society. Unlike one of the other posters here, we would live like Jeremiah Johnson.
It would be a last resort, but we have that contingency. When things go from bad to worse, we simply retreat to our farm, as our greatest strength and resources lay here...food, water, shelter, tools, livestock, etc. We could hold up here for quite a while.
However, when it goes from "fryin'pan into the fire" and we are no longer safe here...(ie from govt search-seizure, roaming hoards of pillagers, insertion of troops, etc.)... we plan to hike it overland to the Blue Ridge Mtns. We know folks there, the land in an area/region, and feel comfortable there. Yes, we would live like pioneers...simple primitive shelter, scavenge and hunt-fish, simple gardens, barter-trade with others up along the "trail"... believe it or not, there already is a subculture up there.
I've said it before, when things get that bad, you are gonna live like"Charlie" in Viet Nam, or the Cozzack/partisans of WWII Russia. Not a glorious life, but you will be free and so very alive.
The key is planning and building those skills now. Practice...read/learn...and live it for a weekend. Assess your strengths and weaknesses. What were your successes and opportunities?

To go off the grid means just live with some other person(s). Pay them an upfront lump sum that in agreement gives you a life time with them. Leave everything else that can link to your new whereabouts behind.

well the best way to dissapear off the grid is to fake your own death. Anything else just looks like you "fell off the grid" that is it looks like you are hidding out somewhere. If you fake your death it is right there at the end of your records DEAD.
