If a doctor wants you or your children to have a vaccine ask them to sign this

I agree that no doctor would ever sign this document, but a carefully placed pile of blanks in clinic and hospital waiting rooms might raise awareness of the potential harm vaccines present.
Also, imagining how many people might pick it up and present it to their doctor makes me laugh.
