NASA said it will take a close look at a small but worrisome gouge in the heat shield of space shuttle Endeavour, discovered in photographs taken as the orbiter arrived on Friday at the International Space Station.

Yet again the tiles are off this time they think its ice that has damaged them ...

there always coming off nowadays the heat shield needs a review methinks.

It may be inaccurate to say "they're always coming off nowadays", becuase I think they have had similar tile damage on almost all of their 100+ shuttle flights. It only gets more attention now because they have a procedure for inspecting the shuttle after launch...they never inspected the tiles in space before the Columbia disaster.
