Im just now looking into this but from the news snippet I heard they said this happened while under the ice. I will submit official news links for verification.
My condoloances tto the British people for this horrible loss.

On the HMS Tireless, a nuclear attack submarine
Saw it on the news this morning.
It was during an exercise with the USN off the coast of Alaska
