Hurricane Katrina?

The report is now released. It was just an accumulation of various bureaucratic factors at several levels, starting at DHS, to FEMA, to the state,
parish, and city levels of government.
Plenty of blame to go around, including some to Congress itself for failure to pass enabling legislation.
The report also lists several positive actions where things went well.
