The only thing I've ever broken was my big toe, which I did by kicking the sofa, the funny thing it was broken for a few days before I realised that the fact it hurt like hell and had swollen could mean it was broken.

Try wrapping something really grippy (sand paper would work but you want to get rid of the itch not the skin
) to the end of a ruler and use that.

A pencil....

The part thats metal will scratch it for you, when I broke my wrist (Attempting to fly in sorts...) I allways used my pencil to scratch an itch

Well, this is how you do deal with it.

You have to ask yourself one question:

"Do I feel lucky?"

Well? Do ya, PUNK?

DLM, get a plastic knitting needle (chopsticks will work too) and bore a heap of holes in the cast...
but not too many to make it unstable. Then get some 'medicated' talcum powder and tip a small amount down the cuff of the cast, and shake it gently. It will reduce sweating under the cast.
*Disclaimer: ask a medico first*

It worked for me.
