No-one has died in a spacewalk, though a few have found out how difficult it can be to do things when you are in an over-pressurised suit.

Been couple close calls - first spacewalk on VOSKHOD 2, March 1965, almost ended in tragedy

Alexey Leonov was almost trapped outside his spacecraft when found that because suit had ballooned out so much
could no longer fit through airlock to get back in. Soviets needed airlock because still used vacuum tubes in electronics
US spacecraft by this time were all solid state/ Soviet electronics needed air to circulate over them to carry off heat

He became completely exhausted trying to squeeze back in - finally had to partially de pressurize his suit by means of valve on arm

Risk getting bends (Soviet atmosphere was 60-40 N2/O2 mix at over 10 psi) US used pure O2 at 5 psi

Leonov was able to de pressure his suit enough to get back in

US found out how difficult spacewalks are, Gemini 9 Gene Cernan (sadly recently deceased) had to abort spacewalk when
became too exhausted and blinded by sweat had to return to capsule.. Richard Gordon on Gemini 11 ran into similar problems and had to cut short spacewalk

If space is such a perfect vacuum why doesn't the space craft basically blow up from the difference in pressure.

NASA has an entire staff of wizards (not the Harry Potter type, but real, actual purveyors of the uncanny arts) to surround all spacecraft with protective fields that reflect or absorb potential threats. These "life shields" are well known in the magic community.

If you have about 3/4 of an hour, there's a fantastic documentary on how difficult it was to make a space suit. NASA was beyond their expertise and ended up contracting out the project to a leading bra maker who knew how to make flexible garments, who sent samples to NASA for rigorous testing.

The funniest part of that video was how the astronauts could pee inside the suit using a condom attached to a bag, but the condoms were falling off some astronauts in testing because of the small medium and large sizes, nobody would use a medium or small condom. To solve the problem, they re-named the three condom sizes to large, gigantic, and humongous, and after that they could get the astronauts to use condoms that fit, since the smallest they had to choose from was "large", and they were OK with using that if it fit.

lol, no pony or 3rd leg sizes?

These guys ride a controlled bomb into space and go to the moon in a can.

They shouldn't worry about peenie sizes.

Worry about where to put them big brass balls.
