Yes it is. Read this article that explains that they have doe this for every President going back to George Washington, how they do it, why they do it, etc. So no, the person doesn't need to lie on a couch. Plenty is observable from people's writings, interviews, TV appearances, etc.

According to the APA it is not ethical to render a diagnosis on someone who you have not personally evaluated:
Since 1973, the American Psychiatric Association and its members have abided by a principle commonly known as the Goldwater Rule,” which prohibits psychiatrists from offering opinions on someone they have not personally evaluated.
