The Hong Kong-based Information Centre for Human Rights and Democracy quotes a close relative as saying Mr Zhao fell into a coma last night.
Mr Zhao served as both the head of the Communist Party and China's Prime Minister for much of the 1980s.
He was ousted in 1989 after opposing the military crackdown on the Tiananmen democracy protests.
He has lived under house arrest ever since.
