Pretty sure, that's just the Shriners being awesome to kids in hospitals...

Doctors put together the course because a current competitor on the show makes prosthetic limbs and has pledged money he may earn to Shriners Hospitals. Kids going through the obstacle course crawl through a tunnel, walk on a balance beam, and pull themselves along the ground. All to the cheers of Doctors and their parents. Finishers get a gold medal each time.
