I was reading a topic and I forgot that I had already flagged the article so I clicked to flag it.
It said there was one less flag so I thought if I clicked it again it would go back to having the flag but all it said was "Fail"
Is that just temporary and I have to go back in later to flag it or something?

This happens to me a lot because I have a terrible memory and forgot I flagged and starred it. Seems temp, I don't know if one is allowed to reload then flag again.

I just tried it with this thread. I flagged. Then refreshed the page. Then unflagged. Then refreshed the page. Then was able to flag again with no problems.

Try refreshing the page.

Try refreshing and waiting a minute. Could be "flood protection" kicking in if you try and flag/unflag/flag too fast. That method usually works for me at least when I get the fail flag.
